Emergency Medical/Permission/Waiver Form
Hello TNC parents!! 
For better safety and better communication in ALL areas of Children’s and Youth Ministry (not just GPS and Youth Group), we would like to have an emergency medical form filled out for ALL kids at TNC… from your newborn using the nursery to your senior coming to Youth Group. You can put all of your kids on ONE form. This is the easiest way to make sure we have all important information regarding your children… from knowing their allergies/medical concerns to having your email/phone numbers to communicate important information!!
